Practice Details
- 100 percent interventional pain role with clinic, ASC procedures, and EMG coverage across Lafayette and Kokomo
- ASC access for spinal cord stimulators, injections, pain pumps, and additional procedures
- Two up-to-date EMG machines in place with scribes and a highly efficient workflow
- Strong referral base with minimal local competition and a physician-owned model built for autonomy
Schedule
- Four-day workweek, 8:00 a.m. to 4:00 p.m.

Compensation & Benefits
- Salary guarantee of $450,000 to $550,000 for two years with RVU bonuses; physicians routinely earn $750,000 or more annually
- ASC partnership available after two years, with a projected two-year payback period
- $20,000 signing bonus and up to $20,000 in relocation assistance

Candidate Profile
- Board Certified or Board Eligible in PM&R
- Strong EMG skills with the ability to perform EMGs independently
